The problem
Physician burnout from after-hours charting, fragmented histories, and appointments that leave no time to reconstruct the story from the chart.
A side-car clinical app that summarises the chart before the visit and drafts SOAP notes for the physician to sign, not a replacement EHR.
30-minute call. No system replacement.
For a clinic, NLP pipelines read historical physician notes before the appointment. Chronic conditions, care gaps, and trends were pulled into one pre-visit brief, so the physician did not rebuild the chart in the room.
Capabilities usedClinical and physician notes, pre-visit workflow abstraction, NLP extraction pipelines.
The same pipelines read examination findings and consultation notes across general medicine, ophthalmology, dental, and aesthetics. Findings were drafted into SOAP notes mapped to the provider’s documentation standard. Nothing was filed until the physician signed.

This application sits beside the EHR. Before a scheduled visit it reads the historical chart, structured lab files, and intake notes, then writes a pre-visit brief and a drafted SOAP note: Subjective, Objective, Assessment, and Plan.
The brief names chronic conditions, missed care gaps, and the trends in recent labs and prior notes, so the physician walks in with the chart already assembled. During or after the consult, ambient audio or typed findings are drafted into a SOAP note that follows that provider’s documentation standard.
The brief and the draft stay in a web view next to the native EHR. The physician reviews both and signs. The master record is updated only after that sign-off, across general medicine and the specialty notes the clinic already keeps.

Chief medical officers, medical directors, and clinical practice leads.
Before a scheduled visit, the pipeline queries the EHR API for historical charts, past diagnoses, and recent labs.
Targeted workflows surface chronic conditions, missed care gaps, and physiological trends from unstructured notes and lab reports.
Real-time or post-encounter audio and text processing drafts SOAP notes mapped to the provider’s documentation standard.
The brief and draft appear in a fast web UI beside the EHR. Explicit sign-off is required before write-back.

The physician. The brief and the SOAP draft stay in the side-car view until they review and sign. Nothing is written back before that.
No. It is a web view that runs beside the EHR you already use. The physician keeps the system of record.
