Interoperability and FHIR EHR middleware
API middleware on FHIR, HL7, and REST that connects third-party apps, devices, and networks to the EHR you already run.
This is custom integration middleware and an interface engine on open standards (FHIR, HL7, REST). It connects third-party applications, diagnostic devices, and external health networks to the provider EHR already in place.
The aim is to meet interoperability requirements and attach modern cloud tools to a rigid EHR without a full-system replacement.
Who this is for
Chief technology officers, directors of IT infrastructure, and health-tech systems integrators.
When it is needed
Interoperability mandates that penalize data blocking, plus the need to connect cloud tools to legacy EHRs without an expensive rip-and-replace.
Technical workflow
API gateway and routing
Central routing for OAuth 2.0 authentication, rate limits, and secure payload formatting on inbound and outbound transactions.
FHIR / HL7 translation
Mapping modules translate proprietary formats or HL7 v2 messages into FHIR resources such as Patient, Observation, DiagnosticReport, and MedicationRequest.
EHR connectors
Adapters for mid-market EHR platforms — including athenahealth, eClinicalWorks, and NextGen — to support read and write without a custom stack for every site.
Monitoring and errors
Live logs of transaction success, dropped messages, and latency across connected clinical systems.
Example use
FHIR interoperability middleware for a hospital and a pharma company
For a hospital working with a pharma company, the team built interface engines and REST API gateways to synchronize orders, lab results, prescriptions, and referrals across heterogeneous systems. Independent facility modules exchanged data with the primary information system with verified message delivery.
Capabilities used: Medication orders and prescriptions, laboratory results, referral management, system configuration, procedure fee schedules.
