A side-car clinical app that summarises the chart before the visit and drafts SOAP notes for the physician to sign — not a replacement EHR.
This application sits beside the EHR. It runs NLP over historical records, structured lab files, and intake notes to produce a pre-visit brief and a drafted SOAP note (Subjective, Objective, Assessment, Plan) before or during the consult.
Nothing is committed to the master record until the physician reviews and signs. The interface is a lightweight web view that runs next to the native EHR.

Human-in-the-loop
The brief and draft appear in a fast web UI beside the EHR. Explicit sign-off is required before write-back.

Ambient summarization and pre-visit intelligence at a clinic
For a clinic, a clinical intelligence module parsed historical notes across general medicine and other specialties. Disparate data points were aggregated into one executive summary, and draft SOAP notes were generated for review. Physicians spent less of the day on charting and more of the encounter on the patient.
